Our PopHealthCare Field Nurse Practitioners perform screenings that matter! Our road warriors will conduct structured and thorough in home evaluations. All our NP's will receive the necessary training and supplies to supplement your existing clinical expertise and judgment and enable you to complete screenings and submit necessary documentation.

Responsibilities:

Provide friendly, safe, comfortable experience for member while performing an exam in member's home. Review and interpret/verify members' medical and social history.
Determine and validate members Primary Care Physician.
Performs appropriate medical screening (Vitals, Monofilament) procedures based on PopHealthCare protocol. Submit completed documentation within 24 hours of exam.
In home screenings are conducted Monday - Saturday with assessments taking 45 to 60 minutes.

Job Requirements:

Valid Advanced Practice State License issued by the State Board of Nursing
Valid accreditation from the AANP or ANCC
Participates in all mandatory compliance and training programs. Current Nurse Practitioner's license in state of practice.
Compliant with OSHA, CLIA, State Board of Nursing and Federal DEA Board of regulations
Current CPR certification
Minimum 1 year clinical experience as a Board Certified Nurse Practitioner. NEW GRADS are WELCOME to apply
Proven ability to independently perform comprehensive adult/geriatric physical exam
Demonstrated ability to recognize conditions, characteristics and behaviors that place adult/geriatrics at risk
Previous experience with proper documentation and charting practices to ensure a full and accurate reflection of patients' health status
Effective verbal communication skills. (Spanish speaking a plus)
Pleasant interpersonal demeanor and ability to work with Medicare population
Committed to excellence, best practices, and superior customer service with every client
Must have valid state driver license and automobile insurance. Must be willing to travel in 50 mile radius.
Must be willing to drive in various weather conditions.
While performing the duties of the job, the NP will be required to meet the demands of the following requirements: sit, stand, talk and hear.Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
